Finding the right CPAP mask is about far more than choosing the newest model or the one with the highest ratings. Comfort, facial anatomy, sleeping position, pressure requirements, and proper fitting all play an important role in successful therapy.

One of the Biggest Misconceptions About CPAP Therapy

One of the biggest misconceptions about CPAP therapy is that there is a single "best" mask.

There isn't.

The best CPAP mask is the one that allows you to sleep comfortably while maintaining an effective seal throughout the night.

Two people with identical sleep studies may leave the clinic wearing completely different masks—and both may be receiving excellent therapy.

Why?

Because successful mask selection depends on far more than simply the diagnosis.

A sleep study can tell us what type of sleep-disordered breathing is occurring, but it cannot tell us which mask will feel the most comfortable on your face, maintain the best seal throughout the night, or allow you to consistently wear therapy.

The right mask is influenced by many factors, including:

  • Facial anatomy
  • Nose shape and size
  • Sleeping position
  • Pressure requirements
  • Mouth breathing
  • Skin sensitivity
  • Facial hair
  • Claustrophobia
  • How much you move during sleep
  • Previous PAP therapy experience

Two people with nearly identical sleep study results may have completely different facial structures, breathing patterns, and comfort preferences.

As a result, the mask that works exceptionally well for one person may be completely unsuitable for another.

The Machine Delivers the Therapy. The mask is where that therapy meets the patient. Choosing the correct interface is often one of the most important factors in achieving long-term comfort, leak control, and successful CPAP therapy.

Don't Choose a Mask Based Solely on Online Reviews

When researching CPAP masks, it's natural to read online reviews.

While reviews can certainly be helpful, they should never become the deciding factor.

A five-star review simply tells you that someone had a positive experience.

What it doesn't tell you is:

  • Whether they slept on their side or back
  • If they breathed through their mouth
  • Their facial anatomy
  • If they had facial hair
  • Their skin sensitivity
  • If they struggled with congestion
  • Their sleeping position

Why It Matters

  • Every face seals differently.
  • Pressure requirements vary tremendously.
  • A great mask at 8 cmH₂O may behave very differently at 18 cmH₂O.
  • Comfort is highly individual.
  • A mask that works perfectly for one patient may leak constantly on another.
  • Personal preference plays a surprisingly large role.

Those factors matter far more than the number of stars beside a product.

A mask with thousands of excellent reviews may be completely wrong for your face, while another with fewer reviews could become the most comfortable mask you've ever worn.

Think of reviews as one piece of the puzzle. They can provide helpful insight into build quality and general patient satisfaction, but they should never replace a professional mask fitting that considers your anatomy, therapy pressures, sleeping habits, and individual comfort.

Even How You Put the Mask On Matters

This surprises many new CPAP users.

A CPAP mask is not quite like putting on a pair of glasses. The way the mask is positioned, where the straps sit on your head, how evenly the cushion contacts your skin, and how much tension is applied can completely change how the mask performs.

After fitting thousands of patients over the past decade, we have found that many mask problems are not caused by the mask itself.

They are caused by how the mask is being worn.

One of the most common reactions to a leak is:

"It's leaking... I'll just tighten it." Unfortunately, tightening the mask is often the wrong response and can make the problem worse.

Most modern CPAP cushions are designed to gently inflate against the skin once airflow begins. This creates a soft, flexible seal.

When the straps are overtightened, the cushion can become flattened and compressed against the face. Instead of floating naturally against the skin, the cushion loses some of its ability to adapt to movement and facial contours.

  • More air leakage
  • Pressure marks
  • Skin irritation
  • Soreness around the nose or face
  • Breakdown of sensitive skin
  • Headgear discomfort
  • Premature cushion wear

  • Removing the mask during sleep

What a Better Fit Looks Like

  • The cushion rests evenly against the skin
  • The straps are secure but not excessively tight
  • The mask is centred correctly
  • The cushion is allowed to inflate
  • Leaks are checked while pressurized
  • The patient is lying in a normal sleeping position
  • The pillow is not forcing the mask sideways
  • Small adjustments are made before tightening further
Quite often, the correct solution is to loosen the mask—not tighten it. A mask that is overtightened can leak just as much as one that is too loose, while also causing significantly more discomfort.

Placement Can Make or Break the Mask

It can sound overly technical to say that the exact placement of a mask matters, but it absolutely does.

A mask positioned only a few millimetres too high, too low, or too far forward may create leaks, pressure points, nasal soreness, skin breakdown, or a sensation that the mask is unstable.

Some masks also have very specific headgear positioning. A patient may naturally place the straps where they have worn previous masks, even though the new design is intended to sit differently.

When the headgear is positioned incorrectly, the cushion may be pulled in the wrong direction. This can create discomfort even when the correct size has been selected.

Proper sizing is important. Proper placement is just as important.

Why We Fit Masks While Patients Are Lying Down

A mask can appear to fit perfectly while a patient is sitting upright in a chair.

Once that patient lies down, several things can change:

  • The jaw may relax or move backward
  • The cheeks may shift against gravity
  • The pillow may press against the mask
  • The headgear may move
  • The mouth may begin to open
  • The tubing may pull on the frame
  • The cushion may seal differently

This is why a proper fitting should ideally be performed while the patient is lying in a normal sleeping position.

Why the Mask Should Be Pressurized During Fitting

A mask that seals at very low pressure may behave completely differently at the patient’s actual therapeutic setting.

For example, a mask that looks perfect at 4 cmH₂O may begin leaking significantly once pressure increases to 14 or 18 cmH₂O.

Pressure changes the way the cushion inflates, how the headgear reacts, and how much force is placed against the seal.

A fitting without therapeutic pressure only tells part of the story. Whenever possible, the mask should be assessed while the machine is running near the pressure range the patient is expected to use.

Sometimes only a few millimetres of adjustment make the difference between repeatedly removing the mask and sleeping comfortably throughout the night.

Nasal Pillow Masks

Nasal pillow masks are among the smallest and lightest CPAP interfaces available. Instead of covering the nose, they use two small cushions that rest at or just inside the nostrils.

For the right patient, nasal pillows can provide a very open and unobstructed experience. They leave most of the face uncovered, make it easier to wear glasses, and can feel less claustrophobic than larger mask styles.

Smaller does not always mean easier. Nasal pillow masks can be extremely comfortable, but the cushion size, angle, headgear position, and amount of strap tension all influence how the mask feels and seals.

A nasal pillow should sit gently against the nostril opening. It should not feel as though it is being forced deeply into the nose, and the straps should not pull the nose upward.

Important Nova Micro fitting note:
The upper straps should sit near the front hairline or top of the forehead, while the rear straps should rest lower, near the base of the skull. Placing both straps too far back can pull the pillows upward, increase nostril pressure, and make an otherwise comfortable mask feel unsuitable.

Preventing Nostril Soreness

Some temporary awareness around the nostrils can occur when first adapting to nasal pillows. Persistent pain, broken skin, or significant soreness should not be treated as something you simply have to tolerate.

Common causes include:

  • Pillows that are too small
  • Pillows that are too large
  • Headgear positioned too far back
  • Excessive strap tension
  • The pillows being inserted too deeply
  • Dry or irritated skin
  • A cushion that has become worn or stiff
  • The mask being pulled by the tubing
Nasal pillows should seal gently—not be wedged tightly into the nostrils. Persistent soreness usually means the size, placement, tension, or mask design needs to be reassessed.

Who May Do Well With Nasal Pillows?

Nasal pillow masks may be an excellent choice for patients who:

  • Prefer minimal material on the face
  • Feel claustrophobic in larger masks
  • Wear glasses while reading before sleep
  • Sleep primarily on their side
  • Have facial hair that interferes with larger cushions
  • Want a lightweight mask for travel
  • Have reliable nasal breathing
  • Prefer a very open field of vision

When Another Style May Be Better

A different interface may be more appropriate for patients with severe or uncontrolled nasal congestion, significant mouth leakage, persistent nostril sensitivity, or discomfort from direct nasal airflow.

This does not mean nasal pillows cannot be used at higher pressures. Many patients tolerate them very well across a wide pressure range. The decision should be based on individual comfort, stability, leak control, and breathing pattern—not pressure alone.

The best nasal pillow mask is the one that feels gentle at the nostrils, remains stable on the head, and maintains its seal without being overtightened.

Under-the-Nose Nasal Masks

Under-the-nose nasal masks, sometimes called nasal cradle masks, rest beneath the nostrils rather than covering the bridge of the nose or sitting directly inside the nostrils.

They offer a middle ground between nasal pillows and traditional over-the-nose nasal masks. They provide less facial contact than a traditional nasal mask, while avoiding the direct nostril contact associated with nasal pillows.

  • No pressure across the bridge of the nose
  • Less direct contact inside the nostrils
  • Open field of vision
  • Comfortable for reading or wearing glasses
  • Less facial coverage than a traditional nasal mask

  • Some top-of-head designs transmit airflow sound through the frame
  • Cushion material affects cleaning and replacement frequency
Under-the-nose does not mean one-size-fits-all. The width of the nose, shape of the nostrils, cushion angle, headgear position, and therapeutic pressure all influence how well a nasal cradle mask performs.

The cushion should sit comfortably beneath the nose without pushing the nose upward or folding the nostrils inward.

If the cushion is too small, it may restrict airflow or create pressure beneath the nose. If it is too large, the nostrils may not remain properly aligned with the openings, leading to leakage and instability.

Stable & Exceptionally Quiet

Memory Foam Comfort

ResMed AirTouch N30i

The ResMed AirTouch N30i uses a memory foam cushion beneath the nose rather than relying entirely on silicone.

In our experience, the memory foam can create a softer and more adaptable seal for many patients, particularly those who find silicone irritating or difficult to seal.

The inverted top-of-head tubing connection can be very convenient for patients who move frequently during sleep because the hose is less likely to lie across the chest or pull directly from the front of the mask.

The memory foam cushion does, however, require more careful maintenance than silicone. It should not be washed or scrubbed in the same manner as a standard silicone cushion, and it generally requires more frequent replacement as the material becomes worn, absorbs facial oils, or begins losing its seal.

Some patients may also notice airflow sound resonating through the hollow frame near the ears. This is not experienced by everyone, but sound-sensitive users should be aware of it.

Why Sizing Matters So Much

Under-the-nose cushions are commonly available in several widths. Choosing the correct size is not simply about selecting the smallest cushion that appears to fit.

The openings must remain aligned beneath the nostrils without pressing against the nasal tissue or allowing the nose to fall too deeply into the cushion.

A Cushion That Is Too Small May Cause

  • Restricted or uncomfortable airflow
  • Pressure beneath the nostrils
  • Upward pulling on the nose
  • Redness or soreness
  • Frequent repositioning
  • Difficulty maintaining a seal

A Cushion That Is Too Large May Cause

  • Air leaking around the nose
  • Instability during movement
  • The nostrils falling inside the opening
  • The cushion shifting on the upper lip
  • Inconsistent seal at higher pressures
  • More frequent awakenings
Fit the cushion to the nose—not the headgear to the leak. If the cushion size or position is incorrect, tightening the straps usually does not solve the underlying problem.

Top-of-Head Tubing: Helpful, but Not for Everyone

Some nasal cradle masks route airflow through hollow side frames and connect the main tubing at the top of the head.

This design can be extremely convenient for active sleepers because the tubing remains above the pillow and can swivel as the patient turns.

However, airflow travelling through the frame may create a resonating or rushing sound near the ears for some patients. The mask may be quiet to a bed partner while sounding louder to the person wearing it.

This does not necessarily indicate a leak or defective mask. It may simply be a characteristic of the airflow travelling through the frame.

Fitting reminder:
Place the cushion directly beneath the nostrils before adjusting the headgear. Confirm that both nostrils are centred over the openings, then make small and even strap adjustments while the machine is pressurized. Avoid pulling the mask upward into the nose.

Who May Do Well With an Under-the-Nose Mask?

  • Patients who dislike pressure across the nasal bridge
  • Patients who find nasal pillows irritating

  • Active sleepers
  • Patients who read or wear glasses before sleep

When Another Mask Style May Be Better

A traditional nasal mask may provide greater stability for patients who experience frequent movement or difficulty maintaining alignment beneath the nostrils.

A full face mask may be more appropriate when persistent mouth leakage, severe nasal obstruction, or inability to maintain nasal breathing interferes with treatment.

The decision should always be based on comfort, anatomy, sleeping position, leakage, and therapy effectiveness rather than appearance alone.

A good under-the-nose mask should feel light and open while remaining stable enough that you are not repeatedly repositioning it throughout the night.

Traditional Over-the-Nose Nasal Masks

Traditional nasal masks cover the nose from the bridge to just above the upper lip, creating a seal around the entire nose without covering the mouth.

Although newer under-the-nose and nasal pillow masks have become increasingly popular over the last several years, traditional nasal masks continue to be one of our favourite starting points for many patients beginning CPAP therapy.

Patients are often surprised by this recommendation.

Many assume that because these masks appear larger, they must also feel more restrictive or less comfortable.

In reality, the appearance of a mask rarely reflects how it feels once therapy begins.

Because the cushion surrounds the nose instead of directing airflow into or beneath the nostrils, many patients describe the breathing sensation as more natural and less concentrated. The airflow feels gentler around the entire nose, particularly during exhalation.

Traditional nasal masks also tend to remain very stable throughout the night. The larger cushion distributes pressure across a broader surface area, making them particularly forgiving for patients who move during sleep.

Don't judge a mask by how it looks. Some of our most hesitant patients end up choosing a traditional nasal mask after trying several minimalist designs. The larger cushion often creates a softer, more forgiving seal that simply feels more natural once the machine begins delivering pressure.

Our clinical perspective. If a patient has good nasal breathing and no significant mouth leakage, a traditional nasal mask is often one of the easiest masks to fit successfully. While minimalist designs continue to grow in popularity, we still find that over-the-nose nasal masks frequently provide the best combination of comfort, stability, durability, and long-term success.
Sometimes the "old-fashioned" choice is still the best choice.

Traditional Full Face Masks

Traditional full face masks cover both the nose and mouth, allowing therapy to continue whether you breathe through your nose, your mouth, or a combination of both throughout the night.

Many people assume that full face masks are only intended for patients with severe sleep apnea or very high pressures. In reality, the decision to use a full face mask depends much more on how you breathe than on the severity of your sleep apnea.

Patients with mild Obstructive Sleep Apnea may benefit greatly from a full face mask, while others with severe sleep apnea may sleep comfortably using only a nasal mask.

The mask should fit the patient—not the diagnosis.

Should You Tighten a Full Face Mask More?

Because full face masks cover a larger area, many new users assume they must be tightened firmly to prevent leaks.

In fact, the opposite is often true.

Modern silicone cushions are engineered to inflate slightly under pressure, creating a flexible seal that adapts to facial movement. Pulling the mask tightly against the face can prevent this cushion from working properly.

A comfortable full face mask should float gently against the face—not be strapped tightly to it. Overtightening frequently creates more leaks, more discomfort, and more pressure marks than a properly adjusted mask.

Hybrid full face masks are one of the biggest innovations in modern CPAP therapy.

Unlike traditional full face masks that seal around both the nose and mouth, hybrid masks sit under the nose while still covering the mouth. This eliminates pressure across the bridge of the nose while maintaining the ability to breathe comfortably through either the nose or mouth.

For many patients, hybrid masks offer an excellent balance between the minimal feel of a nasal mask and the security of a traditional full face mask.

Are Hybrid Masks Better Than Traditional Full Face Masks?
