Eligibility depends on your plan, employer coverage, available benefits, prescription and testing requirements, equipment eligibility, replacement schedules, prior authorization and coordination of benefits.
Some public, restricted or government-sponsored plans may not use the same electronic-claim pathway. Patients remain responsible for amounts not paid by their insurer.

Individuals with significant cardiopulmonary disease, suspected central sleep apnea, hypoventilation, complex sleep-disordered breathing or severe oxygen abnormalities may require physician assessment or in-laboratory polysomnography.
